Transportation Forms

At Fairmont Private Schools, we prioritize the safety and well-being of our students during transportation. We offer two essential documents for parents and students involved in driving or riding with others. Both forms are conveniently available through the MySchoolApp under the "Athletics" tile

Driver Permission Agreement: This form must be completed by anyone who will be driving students.
Passenger Agreement: This form is for students who will be passengers in vehicles driven by others.
